Documents for Summer Grantees
Please find below documents that you will need to complete and submit all the forms (other than the benefits forms which are due sooner) at least 4 weeks prior to your grant start date.
- Grant Benefits form (due by May 1st)*
- Terms and Conditions of the Whitaker Grant (due within two weeks of being notified of your grant status)
- Travel Itinerary Form
- Emergency Contact Information Form
- Bank Information (for direct deposit of your payments as outlined in your Grant Authorization form)
*The grant benefits form is your provisional willingness to accept this award. Please complete and return the form by May 1st.
Final Report Guidelines
Your final payment will be issued only upon receipt of the final report. You can download the guidelines here.
You should submit all documents via the IIE self-service portal, instructions for which were sent out when you were notified of your selection.
Health Insurance Information
The Whitaker International Program provides grantees with health insurance for the duration of their grants. Grantees are covered by Cultural Insurance Services International (CISI) (Gerhard, can you hyperlink Cultural Insurance Services International (CISI) to http://culturalinsurancecome). The CISI brochure provides details about what is covered under the policy along with a claim form.
To print your insurance ID, please create an account by going to culturalinsurance.com and entering your First Name, Last Name, and Birth Date. Select MYCISI at the top right hand corner. You will see the option to create a login.
